What is a patient advocate and what is their role?

A patient advocate is a professional who partners with you to ensure your voice is heard and that you are in control of your own healthcare. They will help you reach your healthcare goals by helping you prepare for medical appointments, attending them with you, and summarizing visits. Additionally, advocates act as a liaison with family members and providers to ensure everyone is informed. They can help coordinate appointments and assist with necessary services or equipment. One of a patient advocate’s main roles is to educate people and provide resources so that they can make informed decisions. 

How can an advocate help with my aging parent who has limited support?

Advocates act as a liaison between you, your loved one and their providers. They are your eyes and ears checking in to confirm your loved one is being heard, getting their needs met and understanding their medical situations. This can provide reassurance and peace of mind to both loved ones and their family members.

How do I make sure my advocate is the right match for me?

Advocates have a variety of backgrounds, experiences, skills and personalities. You should look for someone trustworthy who is not afraid to ask questions and is a good communicator and listener. An in-person interview is always recommended to confirm that it is a good match for both parties.

What if I receive a new diagnosis, am hospitalized, need additional help in my home, or need to find a new living arrangement?

Patient advocates walk with you on your journey wherever you are. When hospitalized they are paramount in communicating with the providers to get the most updated information and ensure your questions are being answered. They also act as liaisons to family members who may not live locally. Moreover the advocate will actively  participate in discharge planning to ensure a smooth and supportive transition. 

What services separate Dawn from other advocates?

My focus is on seniors and care management. With my medical social work background, I have a broad knowledge of various diseases and resources available. I am particularly passionate about Advance Care Discussion conversations. In addition to medical advocacy, I offer “companionship” services at a reduced rate. Finally, I am committed to working with you through all stages and placements of your healthcare journey and, if needed, will be available to help coordinate care in your home and assist you in finding an alternate living arrangement.

What if I am not sure I can afford a patient advocate’s services?

Although it is difficult to put a monetary value on patient advocacy services, advocates can bring relief and peace of mind to clients and family members. Knowing you are not alone and that you have someone working independent of an organization to help you meet your healthcare needs, wishes and goals can be reassuring and priceless. Advocates are always in your corner.
